HSA to carry out investigation following death of teenager in Dublin

The Health and Safety Authority will carry out an examination today into the circumstances surrounding the death of a teenager in Dublin.
Emergency services were called to an equestrian centre in Rathcoole yesterday afternoon after a 15 year old girl received serious injuries during a training exercise.
The teenager was brought to Crumlin's Children Hospital where she later passed away.
Meanwhile an investigation's underway after a teenager was killed in a workplace accident in Co. Waterford.
It happened in Dungarvan yesterday afternoon.
Gardai say the boy was pronounced dead at the scene.
